Chelmsford West PCN is seeking a full-time Pharmacist.
The post holder will be employed by Chelmer Medical Partnership (Humber Road Surgery, Tennyson House Surgery, and Melbourne House Surgery)
The post holder will act within their professional boundaries, supporting and working alongside the practice multidisciplinary teams in a patient-facing role. and take responsibility for areas of chronic disease management within the practices and undertake clinical medication reviews to proactively manage patients.
This role will provide support to general practice staff with regards to prescription and medication queries, help support the repeat prescription systems, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practices.
The post holder will provide clinical leadership on medicines optimisation and quality improvement and will manage some aspects of the quality and outcomes framework and enhanced services. The role is pivotal to improving quality of care and operational efficiencies, so requires motivation and passion to deliver excellent service within general practice.
Ideally the post holder will be a non-medical prescriber; if this is not the case the post holder will be supported to develop their role to achieve this.
We can offer 6 weeks annual leave pro-rata with salary to be discussed at interview.
The ideal candidate will have:
- Pharmacy Degree and significant post graduate experience.
- Independent Prescriber status.
- Excellent clinical and medication knowledge.
- Understanding and experience of working as a pharmacist in primary care.
- Experience of undertaking medication reviews.
- The commitment to continue to learn and develop.
- Excellent inter-personal patient facing communication skills.
- Registration with the General Pharmaceutical Council.
