The Maldon & Witham Primary Care Network (PCN) is a dynamic and forward-thinking PCN committed to delivering high-quality healthcare services to our community. Our multidisciplinary team includes GPs, nurses, pharmacists, social prescribers, and other healthcare professionals working collaboratively to provide comprehensive care.
We are seeking dedicated and experienced Primary Care Physiotherapists to join our team and work across 6 practices. The successful candidates will play a crucial role in providing first-contact physiotherapy services within the PCN, helping to manage musculoskeletal conditions and improve patient outcomes.
Key Responsibilities:
- Conduct initial assessments and provide expert physiotherapy treatment for patients with musculoskeletal conditions.
- Develop and implement individualised treatment plans.
- Work collaboratively with other healthcare professionals within the PCN to ensure integrated care
- Educate patients on self-management techniques and preventive measures.
- Maintain accurate and up-to-date patient records.
- Participate in multidisciplinary team meetings and contribute to the development of care pathways.
Requirements:
- Degree in Physiotherapy and registration with the Health and Care Professions Council (HCPC).
- Experience in musculoskeletal physiotherapy, preferably within a primary care setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
